Objective: Provide a greater copayment spread between preferred and non-preferred brands, while encouraging generic use by reducing the generic copayment to $5.
Description: A three-tier $5/$20/$60 benefit with or without deductibles. Deductible options of $100, $200, $300 and $400 significantly reduce premium.
Market Segment: These five plans will be offered with all large and small group plans, HMO, POS, PPO including Dual Option and YourChoice, CHA Health's Consumer Driven Health Plan.
