We have a wide variety of providers within the CHA Health system. Our goal is to offer comprehensive services to our members, while supplying excellent service to our providers.

It must be working because we have more than 2,500 PCPs in the CHA Health family. In addition, we have 8,500 specialists in the network and more than 140 active hospitals.

Adolescent Immunizations/Well Visit - 14 to 15 Years
At the age of 14 and 15 years of age, adolescents should receive Diphtheria & Tetanus immunizations, if not received at 11-12 year visit.
