HMO Plans
Prior Plan Approval (PPA) is required on all Plans for certain services regardless of whether the provider is participating or non-participating.

Members are not required to select a PCP for any benefit plan offered by CHA Health, but it is recommended.


HMO Plans

The PrimeCare Gold Plan, our HMO option, is easy to use and has no referral requirements for primary or specialty care. When a member enrolls in an HMO Plan, member benefits cover services from participating providers.

Members can refer to their Certificate of Coverage (COC) for further details.
