POS Plans
Prior Plan Approval (PPA) is required on all Plans for certain services regardless of whether the provider is participating or non-participating.

Members are not required to select a PCP for any benefit plan offered by CHA Health, but it is recommended.


POS Plans

Our PrimeCare Platinum Plan, our POS option, combines both HMO savings with an out-of-network option, resulting in more freedom of choice while managing and containing costs. These plans give the member more choices by covering certain services provided by non-participating providers at a lower benefit level. Out-of-network benefits require you to pay substantially more, including deductible, coinsurance and any amount above the eligible expense.
