The easiest way to verify a member's eligibility is to log onto myCHAinfo and click on member eligibility. Identify the provider type and fill in any of the following fields: member ID, social security number of the member, first and last name of the member and the eligibility date.
You can also use CheckPoint™, our Interactive Voice Response system, by calling 859-232-8686 or 800-457-5683, option 2, then option 1. All you need to know is the provider's ID and the member's 11-digit CHA Health Member ID number to use CheckPoint.
