Sign Up for myCHAinfo
  • Simply log onto our web site and select the provider box
  • Next, click the myCHAinfo button, then scroll down to Provider Services and choose to sign up for your provider account
  • Review and agree with the Provider Terms of Access
  • From here, fill in the information required and submit
  • Upon receipt, CHA Health will verify information, authorize your account, and notify you when you are able to begin using your myCHAinfo account
